A waiver and release form is required to be signed by all coaches, volunteers, parents of minor players, and players aged 18 years old. Coverage is contingent upon having procedures in place that require the signing of release and waiver of liability forms by all of the aforementioned parties. Participant Liability Coverage will not apply if this is not in place.
